Late 2012 Player Registration

Registration will be $70.

The registration form and OYSC short form should downloaded and completed prior to coming to registration.  You must bring two copies of each form to registration. An incomplete registration may effect your choice of teams. 

All players must provide a small picture of their face, no hats or sunglasses.  Pictures should be about 1" by 1" of their face.

New players need to provide a copy of their certificate of live birth, birth certificate.

Send the completed OYSC short form, the player registration form and the photo to:
